    There are many over-the-counter products that advertise dramatic results for rejuvenating the skin. Celebrity spokespersons trumpet the benefits of skincare products that turn back the clock, giving the skin a healthy youthful glow, and removing lines and wrinkles in a matter of days. Americans will spend $5 billion on skincare products and cosmetics this year alone. Some of these products are quite pricey, costing hundreds of dollars for a small jar of product that lasts a month. Because the FDA doesn’t monitor or regulate these products, it’s difficult for the consumer to know how much of the benefit they are.

    There are certain ingredients that one should look for in any anti-aging product. The ingredients should address the leading causes of aging of the skin: internal sources such as diet and genetics and external sources such as UV radiation, pollution, wind, and smoking. On the cellular level, the damage from free oxygen radicals to cellular DNA, lipid, and proteins affects the integrity of our skin cells including the production of collagen. Since collagen is the main building block of our skin, decreased collagen production will cause noticeable signs of aging such as wrinkling.

    Some of the more effective antioxidants occur naturally in the skin including vitamins A., C., and E. Our skin also contains coenzyme Q.-10 which neutralizes free radicals. As we age, our production of these antioxidants decreases. In addition, a diet that is lacking in sufficient amounts of these vitamins also contributes to the aging process.
    Vitamin A in its biologically active form is retinoic acid or tretinoin. Vitamin A increases the growth of the epidermis as well as the metabolism of fibroblasts and collagen. They also decrease metalloproteinase which degrades collagen. Topical application of vitamin A products improves fine and coarse wrinkling and pigmentation. One of the more common vitamin A products on the market is Retin-A.

    The Vitamin B group contains certain subgroups which improve wrinkles such as vitamin B 3 (niacinamide) and DMAE. Gels containing both of these drugs improve wrinkles, pigmentation, and yellowing of the skin as well as improve sagging.

    Vitamin C is the most plentiful antioxidant in the skin. These biologically active forms include ascorbic acid and alpha hydroxy acid. Skincare products containing vitamin C often include tea-based products, which seemed to cause less irritation to the skin. Cellex-C is a common brand but there are many more products on the market that contain this antioxidant vitamin C products have been shown to increase blood supply to the skin, increased gene expression of type I collagen, and thus improve wrinkling, roughness, skin laxity, and sallowness.

    Vitamin E is a lipophilic antioxidant. By that, I mean that it scavenges free radicals preventing their damaging lipid cell membranes. Products that contain tocopherols and tocotrienols are Vitamin E products. Studies have shown that vitamin E in combination with vitamin C decreases damage to sunburn cells and thus serves more as a protective agent.

    Coenzyme Q. 10 and Squalene are also lipophilic antioxidants that decrease with age and radiation. A synthetic form of coenzyme Q. 10 is idebenone. Alpha lipoic acid is another free radical oxygen scavenger that increases collagen synthesis.

    Alpha-hydroxyl acids (AHA) thin out the top layer of skin and speed up the normal process of skin cell exfoliation and regeneration. They can also increase the thickness of the skin and improve the quality of elastic fibers and collagen. Many AHA’s occur naturally in foods. Roman women knew the benefits of treating their skin with wine and grapes, which contained tartaric acid, as well as citrus fruits. Ascorbic acid is found in fruits, vegetables Gary products, and tea. Glycolic acid is found in sugarcane and lactic acid is in sour milk and tomato juice. Over-the-counter concentrations of AJ’s are quite low but can be beneficial as a home therapy supplement to in-office chemical peels which are done with a higher concentration.     I see hundreds of patients in my practice every year who want body contouring. By that, I mean liposuction, tummy tucks, and removal of excess skin and fat from the upper arms and thighs. Some of the patients are overweight, and others have lost significant weight through diet and exercise, or surgical procedures such as gastric banding and gastric bypass. Most of the patients follow an exercise regime and diet to some extent. I explain to my patients that surgery is not an alternative to proper diet and exercise. It is not a quick fix. There are some patients who are extremely overweight and need to be “debulked”, to reduce their overall size, so that they can exercise properly. Others are extremely self-conscious about their appearance and would exercise more if they weren’t so heavy.

    It’s important that patients continue to exercise and eat properly in the perioperative period. Having good muscle tone and good cardiovascular capacity will help them through the postoperative period. Proper nutrition is essential to good healing. I encourage my patients to eat a lot of fresh fruits and vegetables, particularly during the summer, as well as have adequate protein intake with fish, meat, and soy products. Multi-vitamins are also important since many patients are lacking trace minerals such as zinc in their diet. Iron supplements are important, especially for women and endurance athletes such as long-distance runners, cyclists, and swimmers. Those athletes tend to be on the anemic side. Being anemic limits the oxygen-carrying capacity of your blood and could affect wound healing.

    My patients who are active exercisers always ask “When will I be able to start exercising after surgery?” I try to encourage them to be up and walking on the day of surgery to minimize the development of venous thrombosis (blood clots) in their legs as well as to improve their breathing and decrease the chance of developing postoperative respiratory problems. More extensive procedures such as tummy tucks will require limiting the exercise routine in the first 10 days. I follow my patients very closely in the early postoperative period and will gauge their progress and allow them to exercise accordingly. It’s important to start slow and gradually increase your exercise routine. I don’t want a patient to put too much stress on a suture line and increase the risk of hypertrophic scarring or wound separation.

    Postoperatively, it is also important to drink a lot of water, to help flush out the anesthetic drugs from your system. Stool softeners and fresh fruits are important in patients taking painkillers like hydrocodone. High-protein diets and diets rich in vitamins C and E. are also important after surgery. I myself eat 5-6 small meals per day instead of large meals. I call it “grazing”, but it keeps your metabolism at a high rate and makes you feel less sluggish.

    I always tell my patients that surgery is only part of the overall plan to improve their appearance. Proper diet, exercise, and a positive attitude are just as important in helping you to look your best and feel your best.

We started life in the postwar World War II era, lived through the Cold War, and lived through the 60s with Vietnam and “Flower Power”. We were the “excess of the 80s”, the “.com era”, and made it to the 21st century. We saw the development of fast food restaurants as a way of eating.

    I think that we Baby Boomers have tried to stay fit and live longer healthier lives than our parents, although we are probably more stressed than our parents. The fitness craze of the 80s has continued with home gyms, better diet regimens, less cigarette use, and the need for better nutrition. When I was younger, 40 was the start of middle age. Baby Boomers now look at 60 as the start of middle age.

    People are learning more about how their cells age and how we age as a whole. We’re learning about antioxidants and what they can do to help repair damage sustained over the years from both internal and external factors. We know about the oxidation of molecules, in our body by free radicals, which are unpaired electrons. Although many of the natural human processes involve free radical formation, there are many free radical formations that are detrimental to our cells. One of the causes of cancer is free radicals reacting with DNA to cause mutations. We use skincare products that contain free radicals to prevent sun damage and slow down the aging process. We eat foods that contain free radicals to help repair processes within our bodies.

    We have seen over the last decade, much news about healing cellular damage with proper nutrition as well as supplements. The number of health food products has increased significantly. Although some profess to improve your health dramatically, many have little or no real benefit. They do not cure diabetes, relieve arthritis, or cure heart disease. The companies that manufacture these products cannot, under US laws, make drug claims that they cure disease. There are anecdotal stories from users, which have helped relieve arthritic pain, help with depression, and provided extra energy, but these aren’t scientifically proven. There are some products though, such as juice from the mangosteen fruit, which has been shown to have a “kill effect” on human cancer cells. Mangosteen fruit contains xanthones, molecules that are known for their strong antioxidant content. They appear to have an anti-inflammatory effect as well as help with many G.I. symptoms such as diarrhea, dyspepsia, and nausea. There are also claims to have antiviral and antibacterial properties.

    As with many of the phytonutrients, since they are not regulated by the FDA, there have been some overoptimistic claims as to their benefit. I asked the listeners to know that these products can’t hurt them and may give them benefits. These products have been available for hundreds of years as folk medicine to the natives, and they swear by their benefits.
